Graduate Practical Nurse Program - Home Care

• Graduate of an approved discipline specific program
• Licensed Practical Nurse (required)
• Less than one (1) year experience

• Perform superior assessment skills, combined with a broad skillset to include management of the ventilator dependent patient, which requires constant oversight and clinical intervention
• Administer intravenous fluids and perform sterile procedures
• Wound management and maintenance of skin integrity
• Recognize and respond to clinical situations appropriately and timely to prevent patient decompensation and injury
• Identify and assess patient needs and clinical changes and accurately report assessment findings in a timely manner to the supervising RN and physician
• Collaborates with patients/families and health care providers in the delivery of health care services to ensure patient progress for effective outcomes
