POPULATION HEALTH - BEHAVORIAL HEALTH DEPRESSION CARE
This position is for a caremanager in the field of behavioral health nursing and utilizes thecollaborative care model f or treating depression and anxiety in primary care.The nurse, in the car e manager role on the collaborative care team, conductspatient contacts primarily through scheduled telephone appointments. Care managementinclu des assessment of medication effectiveness and adherence, providing patienteducation on mood disorders and pain, and brief structured counseling thatfocuses on the patient's own self-management of their depression. Uponreferral from the primary care provider the care manager is responsi ble forconducting initial comprehensive psychological assessments, elici ting patient'streatment preferences and needs, presenting patient cases in the weekly roundsmeeting with the consulting psychiatrist and communi cating those roundsrecommendations back to the primary care provider and then to patient. Patientfollow-up appointments are generally every two weeks where the RNre-administers the PHQ-9, assesses medication effectiv eness and side effects,engages the patient in a therapeutic alliance, an d encourages treatmentadherence and self-management of depression. Skill s in motivationalinterviewing are helpful in guiding the patient in sett ing self-managementgoals.
Thisposition is working with Duke Pri mary Care (DPC) patients via telephone whohave depression and anxiety sy mptoms. DPC is one of the state's largest primarycare physician networks . The DPC network plays a key role in Duke Health'sclinical care and res earch, and reflects Duke's commitmentto providing highquality care to i ts employees and peoples in the surrounding area. DCPcomprises 28 primar y care practices and 5 urgent care practices in sevencounties including Durham, Wake, Orange, Granville, Alamance, Chatham, and Vance.DPC provid ers see more than 600,000 patient encounters annually. Currently we are hiring RN's whohave a high degree of empathy fo r patients who are experiencing symptoms ofdepression and other behavior al health concerns. Patients are referred by theirprimary care provider for telephone support and management of their depressionsymptoms. The RN behavioral health care manager provides patient care viatelephone appoi ntments and via the electronic patient medical recordcommunicates with p rimary care providers any recommendations from theconsulting psychiatris t.
